As an Evergreen designated school in the Michigan Green Schools initiative, we make efforts to reduce waste and limit our consumption. We have mailed you an original Emergency Information Card which must be completed and returned, however, the remainder of the forms can be completed and returned electronically if you have the capability. All forms must be returned by Monday, August 2, 2021.

Enrolled families, choose your child’s grade level below to download the required forms for enrollment and attendance. Forms must be completed and returned to Meadow Montessori School prior to your child’s first day of attendance.
